Standard 1 – Critical Thinking in Nursing Practice

The analysis part includes the evaluation of the information that is relevant and reliable. Here, the example of nursing training that has been used is the wound dressing that is the intervention used in nursing practice (Moorhead, Johnson & Swanson 2014). It is important for the nurse to have proper knowledge about the wounds and they should be able to give immediate care to the patients.  The thinking processes that are implemented in case of the nurses are slightly different from the individuals in other professions that are not based on patient care services (Decker et al. 2013). The management team of the nursing department needs to be precise and to the point. The thinking process of the nurses should be logically supported and they should have the clear idea of what care needs to be given to the patients. The nurses should have a clear approach towards which medical care needs to be given to the patients. The communication skills of the nurses should be good enough to understand the problems of the patients. While giving care to the patients the nurses needs to apply all the attributes of critical thinking (Decker et al. 2013).

For registered nurses, the practice procedure of team is based on the purposeful engagement in the effective therapeutic techniques and building the professional relationships with the patients. The registered nurses establish a perfect relationship with the patients by understanding their needs, comfort and safety and they try to sustain that relationship during the stay of the patients (Burston et al. 2015). They try to conclude their relationships on a very professional note so it affects the patient and they have a good notion about the healthcare organization. They should know their boundaries of their professional and personal relationships (Burston et al.2015). The nurses supervises, coordinates, consults and give referrals to the patients to  establish professional relationships with the patients for getting better healthcare outcomes. They foster safety and learning, which includes being engaged with the physicians to attain better knowledge to help the patients for giving patient-centered care (Fitch, Fliedner & O’Connor 2015). The nurses should participate in the collaborative practices where they have to work as a team. Sometimes, the registered nurses have to lead a in certain scenario they have come across, and then they have to show their knowledge and efficiency as a professional. The nurses should use full range of verbal and as well as non verbal modes of communication for understanding the patient’s problems and their challenges. They must observe the patient’s ways of communication and how these affect their ill health, disability and other factors for their physical and mental ailments. Through the processes of communication the nurses should be able to recognize the several challenges the patients face while doing they come for treatments in the hospital (Fitch, Fliedner & O’Connor 2015). While doing the dressing of any wound it is necessary for the nurses to have a good communication with the patients. The dressing of any wound is very important for the patients as neglecting the wounds can cause severe problems. The duty of the registered nurses is to have accurate details of the wounds that are in the body of the patients and they should keep a record for their problems of the patients (Moorhead, Johnson & Swanson 2014). On occasion, registered nurses may find themselves in the position of being expected to provide nursing care to family, friends or acquaintances. While this is generally not appropriate, due to the inherent conflict of interest present in the relationship, there may be circumstances where such a situation is unavoidable. Prior to entering into a therapeutic relationship under these circumstances, the nurse should ensure that attempts to exercise other options have been exhausted or that other options do not exist. The nurse must acknowledge the presence of an inherent conflict of interest, be aware of the potential difficulties in maintaining professional boundaries between the personal and the therapeutic relationship and actively institute measures to manage the situation. In communication with the client, the nurse must acknowledge that while a social relationship exists, the client can expect to be treated professionally and that information exchanged during the therapeutic relationship will be kept confidential even after the therapeutic relationship end (Fitch, Fliedner & O’Connor 2015). The confidentiality of the patients’ identification should be maintained by the nurses and they should not be revealed to any other external sources. The decision making of the nurses should be very important for the identification of the disease and the effectiveness in their treatments. The use of invasive and the non invasive techniques are very important to maintain the safety of the patients. The nurses should also respond to the changing in the needs of the adults and the families and the career during the illness that are terminal. Through their relationship building skills the nurses needs to recognize the early signs of the any form of disease (Fitch, Fliedner & O’Connor 2015). For instance, if the patients have external bleeding and the external wounds are taken care of but still they are  having certain pain that are internal in nature, can be only detected with good communication process. The internal injuries are thus detected through the process of many tests that are been done after the complaints of pain from the side of the patients and they are only been noticed with the process of effective communication. Therefore, the communication skills and the en engagement into proper therapeutic cores and techniques are very important for effective of practice for the registered nurses (Fitch, Fliedner & O’Connor 2015).

Standard 2 – Building Professional Relationships for Optimal Care

The third standard talks about providing safety to the patients and responsiveness of the nurse in their good quality of practice. The registered nurses should provide and deliver quality services to the patients. They should also take this into account that they needs participate in such a way that they do not disobey the ethical rules of the patients and the patients have a peaceful environment to the patients. The patients’ information in healthcare sectors is stored in forms of data (Ross-Kerr & Wood 2014). The analyses of these data are done by the registered nurses. Their work is to analyze the data and give the correct feedback to the patients and through their skills of effective communication. The nurse always conducts the assessments that are holistic in nature (Moorhead, Johnson & Swanson 2014). The registered nurses always provides good care in context to healthcare services and always have believes in ethical and goal oriented plan of action (Ross-Kerr & Wood 2014). They are based on the process of comprehensive assessment techniques that are systematic in nature. The comprehensive safety measures that are used by the nurses for dressing the wounds play a very important role in giving a quality services to the students. In previous cases it had been observed that the negligence of the nurses can harm the patients to a severe level. The negligence of the patients can harm the professional value of the nurses. A recent case of needle based injuries proves that (Ross-Kerr & Wood 2014). During the dressing of the patients, the needles for giving the IVs are not properly sterilized, and then the patient can go through a severe case of needle injuries. This may cause septicemia to the patients. The challenges that the healthcare organization has to face due to the carelessness of the nurses can degrade the reputation of the organization in the future.  . The malpractice of the nurses can cause the breaking of the ethical issues that are applied for healthcare organizations (Ross-Kerr & Wood 2014).The nurses have their own boundaries and the policies developed in the organization with respect to the ethics aware them about their limitations as a healthcare service person. The registered nurse delegates several services to the nurses who are freshly enrolled and also briefs them about the several roles of the nurse in the clinical and non clinical areas (Moorhead, Johnson & Swanson 2014). The nurses also effectively manages and supervises their subordinates and they take care of the timely directs them for the delivery of good quality of work to their patients. The main objective of the registered nurses is to give effective care to the patients when needed and they take care of their safety and maintain their functions and professionalism (Moorhead, Johnson & Swanson 2014). They use of appropriate processes for the identifying of the reports that are analyzed according to the rules of the expected standards (Moorhead, Johnson and Swanson 2014).

Standard 3 – Ensuring Patient Safety and Delivering Quality Services

The main concern of the nurses in a healthcare sector is to evaluate the outcomes of the informed practice. The registered nurses have the responsibility to evaluate the practice based technologies and they usually practices according to the goals and priorities as a physician and healthcare professional (O’connell, Gardner & Coyer 2014). As a registered nurse, they have to evaluate the wounds of the patients that are to be taken care of.  For this, the wounded patients are needed to attended first and should be given highest priority. The responsibilities of the nurses includes, monitoring and having a check that what extra care or services that needs to be given by the patient that they are attending (O’connell, Gardner & Coyer 2014). In case of severe patients whose wounds are very deep and are critical, for them extra care monitoring is required. The normal patients need monitoring on a daily basis or on the interval of 13 to 14 hours. However, the patients who are critical, needs hourly monitoring and they also needs extra care and quality treatments (O’connell, Gardner & Coyer 2014). The evaluations of the condition of the patients are very important. The evaluations are done on the basis of their improvements or their defoliation of the health on a daily basis. The healths of the patients are the most important aspect that is needed to be recorded by the nurses (O’connell, Gardner & Coyer 2014). The main responsibility of the patients is to improve their working nature and their workings aspects include have the correct records of the patients and their daily progress in heath. The evaluation of the patients needs to be quick and correct (O’connell, Gardner & Coyer 2014). The plan for evaluating the patients needs to be revised in a proper gap of months. The continuous revision in the course of plan in evaluating the health structure of the patients helps the standardization of the process of evaluation. The evaluation process determines the documents and the communication of the further priorities of the proper goals and the outcomes of the patients. The relevance in the evaluation of the patients is to be maintained by the registered nurses. It is the responsibility of the nurses to have a proper evaluation report of each patient that they are responsible of they needs to be proper in the ways and techniques they apply for giving good quality and goal oriented care to the patients (O’connell, Gardner & Coyer 2014). Therefore, it is very crucial for the registered nurses to know about the proper checklist of the patients and have a in depth monitoring of the patients and their health.
