Trudy Green is a 24-year-old woman who lives in 1208 Happy Valley. She lives with her partner in a house that they have bought. She is a happy person and is quite regular with her checkups. She has childhood asthma from very small age and follows an action plan for it. However, she has not visited hospital for any emergency regarding her asthma. She often suffers from colds and flu quite frequently and suffers from sinusitis with yellowy brown discharge most of the times. She also has to go though frequent attacks of asthma mainly because of her cold and flu symptoms. She also feels stressed sometimes that result in occurrence of ulcer in the mouth. Sometimes she also suffers from headache mainly due to stress but she takes panadol that helps her to feel fine. She has good values in her life and sends Christmas and Easter with her family. She believes that everyone should get the scope for research potential in education and mainly teaches fro standards 6 to standard 12. She is mainly seen to suffer from allergies like cats, flus, grass and many others. She is mainly seen to spend her free time by watching television on couch as well as dancing in the weekends. However, she has issues with her vision where she has astigmatism and sometimes suffers from dizziness as well as redness of her eyes. Altogether, she is seen to be a happy person who has the urge to live better quality life and aspire to look forward in life.

From the entire psychosocial theory put forward by Erikson, it can be seen that the client is coming under the stage numbered 6 called the Intimacy versus isolation. This stage mainly initiatives the period of early adulthood when people begin to explore their personal relationships. According to the researcher, it is important for individuals to develop close as well as committed relationships with other people. He has also stated that people who are able to succeed in this step will be able to develop relationships that are not only enduring but is also secure. It is usually stated that every step in this theory becomes successful only when the similar success is obtained from the previous step (Darling et al. 2015). It is stated that those individuals who are seen to explore independence in the previous step and are able to develop a sense of self are those who become successful in the later step. Those who receive proper encouragement as well as reinforcement through personal exploration are mainly seen to emerge put with a strong sense of self as well as feelings of independence as well as control. It might be true that the client has gone through stage 5 sufficiently for which she had been able to successfully establish a happy relationship with her partner. With the help of family members and the support of her education by achieving masters, she has been able to develop a strong sense of personal identity that has made her comfortable and self-sufficient (Darling, 2015). Therefore, these attributes had helped her to build an intimate relationship with her partner. Researchers are of the opinion that individuals who have poor sense of self are more likely to have less committed relationships. They are more likely to suffer from different types of emotional isolation, loneliness as well as depression. It has been seen that the client was a very happy soul and she was quite comfortable with the condition that she was living in. She had no major mental health disorder and was living life satisfactorily. Researchers say that when successful resolution of this stage takes place, it results in the virtue known as love. It is mainly marked by the ability of the individual to form long lasting as well as meaningful relationships with people (Dunkel and Habrke 2017). The client was seen stating that she performs some part of the household work like coking, shopping and others ad rest part are done by her partner. Moreover, she also had positive answers in the relationship section stating that she was quite successful in establishing effective relationships. This stability had a positive impact on her life and therefore she has stable mental health.

Importance of Establishing Intimate Relationships

From the entire interview, it is seen that the client has an improper habit not taking enough fluid throughout the day. Moreover, she is seen to release her bowel every three days that is not at all normal for a regular human being. Researchers are also of the opinion that very normal human being should drink at least eight ounce or about 2 litres of water every day (Nauman et al. 2016). Many negative effects may result from low fluid intake. This is loss of concentration and there is also interference of blood circulation (Emannuel et al. 2017). There is decline in the lung function along with the loss of kidney renal function. It also increase palpitations in heart, declining in muscle function, balance disorder, body feels hot or sultry, gets easily tired, head feels dizzy, sleepy, dry eyes and many others. The client has indeed stated that she feels blurred and dizzy sometimes mainly due to low fluid intake or low water intake and this factors are hence related with the potential problem of less fluid intake. As she will age, this habit of low fluid intake will ultimately result in elimination and bowel movement problem causing constipation. Constipation is very common in the old age and her habits would ensure the occurrence of the potential issues associated with constipation (Schimdt et al. 2015). It is seen that the diet she takes is high on proteins, as it comprises of turkey, chicken and others with very few food items high in fibres like green goods and vegetables. Although she takes tomatoes and pumpkins in diet but the foods are not high in fibres. Therefore, lack of proper diet high in fibre and lack of fluid intake might result in constipation when she gradually ages and this might give him a very hard time. Therefore, her activities of daily life like elimination would be affected.

One of the most important health assessments that has been found is the repeated occurrence of chronic sinusitis in the client. It is the condition in which the cavities if the individual patients around the nasal cavities become inflamed as well as swollen. In most of the cases, these occurrences are seen to occur for about 12 weeks despite treatment attempts. Many researchers also state this disorder as the chronic rhinosinusitis. This condition is mainly seen to interfere with that of the drainage and even results in the development of the mucus build-up (Bachert et al. 2016). Under this situation, it is seen that the breathing through the nose becomes very difficult for the patients. In this case of the client as well, it was seen that she has repeatedly stated of her uncomfortable condition when she has runny nose where browny yellowish discharge is mainly seen to occur. This disorder mainly takes place with the issue of infection. The infection is mainly seen to grow in the sinuses as well as the nasal polyps or even due to deviated nasal sputum. Researchers are of the opinion that there are certain risk factors that result in the occurrences of the disorder (Rusmik and Soler 2015). Nasal passage abnormality, aspirin sensitivity, immune system disorder, asthma, hay fever and allergic conditions, regular exposure to pollutants all these result in occurrence of chronic sinusitis. In the case of patients, it was seen that asthma and hay fever or allergic conditions were present and therefore the cause of the occurrence of sinusitis in the patient could be understood (Rezai et al. 2016). Many of the researches say that nasal polyps as well as deviated nasal septum may also contribute to the development of the disorder.

A number of facts are learnt from the assignment. It helped to show how mental health condition results in development of high quality life of patients. It was noticed that the patient had successfully completed each stage of the life cycle with supportive family members and supportive environment.  Therefore, she had been able to develop stable life conditions and are also able to establish intimate relationship with her partner successfully. Moreover, this assignment helped me to understand that taking on proper health assessment of the patient would help the professional to understand many important details of the patient. Such details help professionals to establish the links between the different disorders and hence help to understand the care plan and interventions that need to be taken for the patient.
